I declare that the contract with the above mentioned references has been entered into on my name and that I thus have the authority to authorize edpnet to terminate it in my name.I acknowledge that edpnet's responsibility under "Easy Switch" is limited to the sending to my current operator at the moment that I mentioned here above of a request for termination of my contract and that edpnet is not responsible for its effective termination nor can be held liabile for its consequences such as, but not limited to, the no longer functioning of services from my current operator after migrating my internet connection to edpnet.
I'm aware that I will maintain access to my current email address(es) for 18 months and that my website will be accessible for at least 6 months from the date of termination of my current contract, that is if my current operator has signed the ISPA Code of Conduct as referred to in article 121/1 of the Act of June 13 2005, and if my current internet subscription includes e-mail addresses and / or web space.
I know that if a technician's visit is required for the activation of my internet service, I will be entitled to compensation in case of non-appearance by the technician within the agreed timeframe as stated in article 19 of the Royal Decree of 6 September 2016, and that I can submit a written request theretofore via info@edpnet.be or via the creation of a ticket.
I confirm that I have visited the "Easy Switch" web page of my current operator and that I am aware of the fact that, under penalty of being due interest for late payment or compensation, I am obliged to fulfill all my existing contractual obligations with my current operator, such as, but not limited to, the payment of damages in case of early termination of the contract during the first 6 contract months, the payment of any residual value of a device or the return of appliances on loan.
I accept to be fully responsible for the accuracy of the information entered above and that incorrect information provided by me may cause edpnet to not be able to complete the "Easy Switch" procedure.
